Assuming you have called and scheduled your appointment for either an in-office, in-person or over-the-phone future appointment, there are steps you can take to help you get more from your appointment.
Here's a few tips to help you prepare:
Be prepared to share your provider and/or specialist information, including:
Primary Care Provider (your family doctor)
Specialists (how many visits per year?)
Preferred Dental provider
Preferred Optometrist (Vision) provider
Preferred Pharmacy
Preferred Hospital
Compile a list of your Prescription Drugs, including:
Drug name and size
Dosage (quantity per day/week/month)
Refill Frequency (30 days, 90 days, etc.)
A few questions I may ask to help you narrow down your plan choice:
What do you like and/or dislike about your current plan?
Do you have benefits that are more important to you compared to others?
If your preferred provider is out-of-network, would you consider an in-network provider to get improved benefits and/or lower out-of-pocket co-pays?

Do not share sensitive personal health information via email (e.g. Social Security number; Medicare/Medicaid info, etc.) Should that information be required for eligibility or enrollment purposes, it will be requested in person or over the phone.
