Understanding Your Medicare Options: Medicare Supplement vs. Medicare Advantage

When you become eligible for Medicare, one of the most important decisions you'll face is how you want to structure your coverage. Original Medicare (Parts A and B) covers a large portion of your healthcare costs — but not all of them. That's where Medicare Supplement Insurance (Medigap) and Medicare Advantage (Part C) come in.

Two Paths. One Decision.

There is no single "best" option. The right choice depends on your health needs, preferred providers, budget, and lifestyle. Understanding how each path works is the first step.

Option 1: Original Medicare + Medicare Supplement (Medigap)

Medicare Supplement Insurance, commonly called Medigap, is private health insurance designed to help pay costs that Original Medicare doesn't cover — such as coinsurance, copayments, and deductibles. Medigap plans are standardized by CMS and identified by letter, meaning the benefits for each plan letter are identical regardless of which insurance company sells it.

This path gives you access to any provider in the United States that accepts Medicare, with no referrals required to see a specialist. It's often the preferred choice for people who value flexibility and predictable out-of-pocket costs — particularly those who travel frequently or have established relationships with specific providers.

One important note: Medigap does not include prescription drug coverage. If you choose this path, you will need to enroll in a separate Part D plan for drug coverage, and a standalone dental and vision policy if those benefits matter to you.

Option 2: Medicare Advantage (Part C)

Medicare Advantage is an alternative way to receive your Part A and Part B benefits through a private insurance company approved by CMS. These plans must cover everything Original Medicare covers, and may include additional benefits that Original Medicare does not.

Medicare Advantage plans typically operate within local provider networks (HMO or PPO), and most include an annual out-of-pocket maximum — a cap on your total cost exposure for the year. Monthly premiums vary depending on your area so be sure to confirm actual plan costs before enrolling. Expect to pay for services as you use them.

This path tends to appeal to people who are comfortable staying within a local network of providers, and value the convenience of having most coverage bundled into a single plan.

Which Path Is Right for You?

A few questions worth considering as you evaluate your options:

  • Do you prefer the flexibility to see any provider in the U.S. without a referral?

  • Are you comfortable staying within a local network of doctors and hospitals?

  • Is a predictable monthly premium cost more important to you?

And if you're considering Medicare Advantage, are you aware that switching to a Medigap plan later may require medical underwriting — and your premium at that time will reflect your age?

Taking time to think through these questions can help guide your decision. For personalized guidance on which path is right for your specific situation, consult your State Health Insurance Program (SHIP), visit Medicare.gov, or call 1-800-MEDICARE. A licensed insurance professional can help you evaluate your options based on your individual needs and circumstances.
